HYPOTHYROIDISM SHORT NOTES

  • Hyposecretion of thyroid hormones T 3 and T 4
  • Hypothyroidism in childhood is called cretinism or congenital hypothyroidism
  • Hypothyroidism in adult called myxedema, more occur in female.

CAUSES

PRIMARY HYPOTHROIDISM

  • Congenital defect of thyroid gland
  • Iodine deficiency (simple goiter)
  • Surgery and radiation of thyroid gland
  • Excessive intake of anti thyroid medication like propylthiouracil
  • Hashimato thyroiditis (autoimmune illness that cause destruction of thyroid gland)

SECONDARY HYPOTHYROIDISM

  • malfunction of pituitary gland
  • Tertiary hypothyroidism
  • If hypothalamus fails to produce Thyroid releasing hormone

SIGNS AND SYMPTOMS

  • weakness, intolerance to cold, weight gain, bradycardia, constipation, loss of memory, menstrual disturbances, myxedema, goiter, lethargy, dry skin and hair

DIAGNOSIS

  • Decreased T3 and T 4 hormone level
  • Increased TSH level
  • Increased lipid and cholesterol level in blood
  • Decreased radioactive iodine-131 uptake

TREATMENT

  • Monitor vital signs
  • Administer levothyroxine sodiun
  • Instruct the client to take low-calorie, low cholesterol and low saturated diet
  • Provide warm environment to client
  • Monitor for overdose of thyroid medications characterized by tachycardia, chest pain, restlessness, nervousness and insomnia.

MYXEDEMA – Generalized puffiness and edema around the eyes and face.
