HYPERPARATHYROIDISM
- Hypersecretion of parathyroid hormone by the parathyroid gland
SIGNS AND SYMPTOMS
- Hypercalcemia and hypophosphatemia
- Fatigue, muscle weakness, skeletal pain and tenderness, anorexia, weight loss, constipation, hypertension, cardiac dysrhythmias, renal stones
TREATMENT
- Monitor intake and output and for signs of renal stones
- Monitor for skeletal pain. Move the client slowly and carefully to prevent pathological fracture
- Encourage fluid intake
- Administer furosemide(Lasix) to lower calcium levels
- Administer phosphates, which interfere with calcium reabsorption, as prescribed
- Administer calcitonin to decrease skeletal calcium release and increase renal excretion of calcium
SURGICAL MANAGEMENT
- Parathyroidectomy
PARATHYROIDECTOMY
- Removal of one or more of the parathyroid glands
PRE-OPERATIVE PREPARATION
- Monitor for electrolytes, phosphates and magnesium level
POST-OPERATIVE MANAGEMENT
- Monitor for respiratory distress
- Place a tracheostomy set, oxygen and suctioning equipment at the bedside
- Monitor vital signs
- Position the client in a semi fowler position
- Assess neck dressing for bleeding in posterior side
- Monitor for laryngeal nerve damage
- Administer calcium and vitamin-D as prescribed
